Please call our office at (559) 226-1316 to schedule an appointment. You can also email us at firstname.lastname@example.org to request a call to schedule an appointment. Please provide your phone number, as well as your name and the type of insurance you will be using. This will help us get back to you more quickly.
After scheduling your appointment, you can either have the receptionist email you our paperwork, or you can come by the office before the day of your appointment and pick them up in person. If neither of these options work for you, you can also complete your forms on the day of the appointment. If you choose to do this, we strongly recommend arriving least ninety minutes early for your appointment. If your paperwork isn't finished by the time your appointment is supposed to start, we will not be able to see you and your appointment will have to be rescheduled.
